Nutrition and Chronic Disease Prevention (NCDP) actively coordinates and facilitates opportunities for coalition membership, community engagement and partnership through collaborative efforts related to chronic disease. In 2015, HCPH Nutrition and Chronic Disease Prevention was awarded a four-year grant to address underage drinking and prescription drug misuse and abuse in youth and young adults (12-25) in Harris County.

At this time, we are actively recruiting members to participate in the Partnership for Success Coalition. Through this new coalition, a sustainable strategic plan will be developed utilizing volunteers for activities, maximizing community mobilization through joint community action, and engaging in broad community actions to change social policy.
